Center for Health and Disease Management (HDM) capitalizes upon Cleveland's access to some of the best hospitals and research universities in the country. HDM is the process of holistically looking at a patient's family and medical history to predict and prevent chronic illnesses rather than using a model of "seeing and treating" diseases.
